Report a Discrimination Concern
How to Report a Discrimination Concern
If you believe you, a third party, or a specific class of persons was discriminated against by Cedars-Sinai based on race, color, or national origin in violation of Title VI of the Civil Rights Act of 1964, you (or your representative) have the right to file a complaint with Cedars-Sinai’s Corporate Integrity Program. You may also file a complaint with the Corporate Integrity Program if you believe unlawful discrimination occurred based on disability, age, sex, or other personal characteristic.
The complaint generally must be filed no later than 180 calendar days from the date that the affected individual knew the alleged discrimination occurred. Your complaint must also make a clear allegation of discrimination on a basis prohibited under the public civil rights laws and provide sufficient information to conduct an investigation.
The preferred method is to file your complaint in writing using the Public Civil Rights Complaint Form which can be accessed by clicking on one of the links below:
- Public Civil Rights Complaint Form — English (PDF)
- Formulario de quejas sobre derechos civiles — Spanish (PDF)
- Анкета для подачи жалобы о нарушении гражданских прав — Russian (PDF)
- فرم شکايت حقوق مدنی عمومی — Farsi (PDF)
The completed form should be mailed or faxed to the following:
Public Civil Rights Coordinator
Corporate Integrity Program
8700 Beverly Blvd.
Los Angeles, CA 90048
Fax: 323-866-7871
It is against the law for Cedars-Sinai to retaliate against anyone who files a complaint or cooperates in the investigation of a complaint.
You also have the right to file a civil rights complaint with government agencies such as:
Michael Leoz, Regional Manager
Office for Civil Rights
U.S. Department of Health and Human Services
90 Seventh St., Suite 4-100
90 Seventh St., Suite 4-100
950 Pennsylvania Ave. NW
Civil Rights Division
Disability Rights Section — 1425 NYAV
Washington, DC 20530
Federal Transit Administration
Office of Civil Rights
1200 New Jersey Ave. SE
Washington, DC 20590
- California Department of Fair Employment & Housing
- Any federal agency that provides financial assistance to Cedars-Sinai
If you would like to file a civil rights complaint with one of these agencies, please visit the agency’s website for further details and guidance.
If you file a complaint with Cedars-Sinai and a government agency simultaneously, the government agency complaint will supersede the Cedars-Sinai complaint, and the Cedars-Sinai complaint procedures will be suspended pending the agency’s investigation and findings.
Cedars-Sinai Public Civil Rights Investigation Process
Within 14 calendar days of receiving a public civil rights complaint, Corporate Integrity will acknowledge receipt of the complaint and begin its investigation. If the complaint is not based on civil rights matters handled by Corporate Integrity, it will refer you or your complaint to the appropriate Cedars-Sinai person or office for follow-up.
Within 60 calendar days of receiving the complaint, Corporate Integrity will make every effort to complete the investigation and send you a response letter stating its findings or, if applicable, indicating that the results of the investigation may not be legally disclosed.
If the investigation finds that the complaint warranted follow-up actions, the response letter will explain the follow-up steps that Cedars-Sinai has taken or will take to address the allegation of discrimination.
If the investigation finds that no unlawful discrimination occurred, the response letter will inform you of your right to appeal the decision, within 60 calendar days of when the response letter was sent, by submitting additional information to Corporate Integrity that would lead it to reconsider its conclusion.
If you submit an appeal to Corporate Integrity, it will make every effort to review the additional information and respond to you within 30 calendar days.
