Hypertension Monitoring Program

TERMS OF USE 

1. ACCEPTANCE OF TERMS

By enrolling in, accessing, or using the Hypertension Patient Monitoring Program (“HPM Program” or “Program”), you (“Patient,” “you,” or “your”) agree to be bound by these Terms of Use (“Terms”). If you do not agree with these Terms, the HPM Program is not a good fit for you and is not a viable option.

2. PROGRAM DESCRIPTION

The HPM Program is designed to assist in the remote (virtual) monitoring of your blood pressure and pulse data. The Program facilitates the input of your blood pressure and pulse readings into the CS Connect app. If your reported values are flagged as abnormal, you will be asked to schedule or initiate a visit with your designated provider or care team based on the detected severity. Your care team will then be able to view your input and advise you on next steps.

The HPM Program features include:

  • Blood pressure and pulse data entry functionality within the CS Connect app
  • Automated alerts for abnormal input integrated with the CS Connect app
  • Previous blood pressure  data reported  by you to the Program
  • Healthcare provider assessments during a CS Connect visit and personalized follow-up protocols
  • Healthcare provider assessments during a CS Connect  visit and personalized follow-up protocols
  • Integration with other CS Connect features and external devices, if applicable (coming soon)

The Program may utilize artificial intelligence (AI) tools for data analysis, alert generation, and support functions. These tools and the Program are intended to support your care team and DO NOT substitute regular medical consultations, clinical judgement, emergency services, or your provider’s direct medical advice.  The Program does not provide real-time monitoring of life-threatening events or immediate medical intervention.

The Program does not provide blood pressure monitoring devices. You are responsible for obtaining, maintaining, and operating your own FDA-approved blood pressure monitoring device, obtained from an external source.  

The Program is accessed through your existing CS Connect app account for virtual healthcare services. The Cedars Sinai Mobile Applications End User License Agreement (“CS Connect Terms”) governs all aspects of your use of the Program and the CS Connect app, including but not limited to: user conduct, app usage requirements, technical requirements, data handling, privacy practices, disclaimers, liability limitations, dispute resolution, and general platform functionality. These Terms address only the specific protocols, device responsibilities, and healthcare provider obligations unique to the Program that the CS Connect Terms do not otherwise cover.

3. ELIGIBILITY AND ENROLLMENT

3.1. Eligibility Requirements.

To participate in the HPM Program, you must:

  • Have been diagnosed with hypertension or elevated blood pressure
  • Have an active CS Connect account 
  • Possess the technical capability to use your own monitoring equipment
  • Own and operate an FDA-approved blood pressure monitoring device
  • Agree to these Terms

3.2. Enrollment Process

Enrollment requires activating the HPM app notification features within your existing CS Connect account, completing all required forms, acknowledging these Terms, and confirming that you have access to a suitable blood pressure monitoring device. As mentioned, we do not provide, maintain, or warrant any monitoring devices.

4. PATIENT RESPONSIBILITIES

4.1. Data Entry and Device Responsibility.

You agree to:

  • Obtain and maintain your own FDA-approved blood pressure monitoring device
  • Take blood pressure readings using your  device at the prescribed intervals directed by your CS Connect care team.  and record the data accordingly
  • Verify the accuracy of the data entered into the Program
  • Follow Program recommendations, including—but not limited to—seeking care either immediately or by scheduling a visit (depending on guidance) and reporting values more frequently 
  • Ensure your personal device is properly maintained and calibrated
  • Report any device malfunctions directly to the device manufacturer. The Program is not responsible for device issues, including but not limited to service malfunction, inoperability, software operating system incompatibility, and connectivity issues   
  • Keep your personal devices secure and prevent unauthorized use
  • Comply with all provisions of the CS Connect Terms
  • Use the CS Connect app and HPM feature only for authorized purposes

4.2. Data Accuracy and Entry Responsibility

You are responsible for:

  • Providing timely, ongoing, and accurate health information through the CS Connect App
  • Taking readings in accordance with the Program and your CS Connect provider’s instructions
  • Promptly reporting to your CS Connect care team changes in your health status, symptoms, or medications
  • Following up on recommendations

You understand that the accuracy and completeness of the data entered is solely your responsibility and that errors or omissions may affect the care provided. You also understand that the data, information, and images you provide through the HPM Program may be included in your medical record.

4.3. Communication

You must:

  • Respond promptly to communications from your CS Connect team
  • Follow up on abnormal readings as directed
  • Attend CS Connect scheduled appointments and consultations
  • Maintain up-to-date contact information in your CS Connect account

5. YOUR CS CONNECT CARE TEAM RESPONSIBILITIES

During your next visit with your CS Connect  provider, your CS Connect provider will:

  • Review blood pressure data that falls outside the normal range as entered through the HPM feature and provide treatment recommendations based on this data
  • Maintain documentation of assessment and treatment recommendations
  • Provide referrals for in-person follow-up and other resources, as clinically indicated.

6. EMERGENCY DISCLAIMER AND PROGRAM LIMITATIONS

6.1 NOT FOR EMERGENCY USE

THE HPM PROGRAM IS NOT INTENDED FOR EMERGENCY SITUATIONS AND MUST NOT BE RELIED UPON FOR URGENT OR EMERGENCY MEDICAL NEEDS.

6.2 Seek Immediate Care

In an emergency, you should immediately contact 911 or go to the nearest emergency room.

6.3 Abnormal Symptoms

Regardless of your blood pressure readings, if you experience any abnormal symptoms such as chest pain and shortness of breath, or feel generally unwell, you should initiate a visit with your healthcare provider immediately. Do not wait for the Program to escalate results or for a response from your care team through the Program.

6.4 Response Time Limitations

Your CS Connect care team does not review your data in real time. Your data will be reviewed only during a visit with your CS Connect provider

7. TECHNOLOGY AND DATA

7.1 Technical Requirements

The HPM Program requires:

  • Your own FDA-approved blood pressure monitoring device (not provided by the Program)
  • An active CS Connect app account
  • Reliable internet connectivity for app access
  • Smartphone with the CS Connect app installed
  • Basic technical proficiency for app navigation and manual data entry

The CS Connect Terms govern all other requirements and terms, including internet connectivity, device compatibility, and app functionality.

7.2 Data Privacy and Security

Your health information privacy rights and how your information is used and disclosed are described in the Notice of Privacy Practices available at

8. PROGRAM MODIFICATIONS AND TERMINATION

8.1 Program Changes

We reserve the right to modify the HPM Program and these Terms at any time, in our sole discretion, with or without prior notice. Modifications may include, but are not limited to:

  • Changes to program features, functionality, or availability within the CS Connect app
  • Updates to clinical protocols or alert thresholds
  • Revisions to patient responsibilities or provider obligations
  • Changes to eligibility requirements or enrollment processes

Notice of Changes: We will provide notice of material changes through one or more of the following methods:

  • Email notification to your registered email address
  • In-app notification through the CS Connect app
  • Posted notice on our website or within the CS Connect app
  • Direct communication from your healthcare provider

Effective Date of Changes: Unless otherwise specified, modifications to these Terms will become effective immediately upon posting. Your continued participation in the HPM Program after the effective date constitutes acceptance of the modified Terms.

Right to Discontinue Participation: If you do not agree to any modifications, you may discontinue your participation in the HPM Program at any time by notifying our care concierge team through the CS Connect app or by  phone.  Alternatively you can turn off your Program notifications in the CS Connect app .

8.2 Termination

You or your CS Connect provider may terminate your participation in the HPM Program at any time. Reasons for termination may include:

  • Non-compliance with program requirements
  • Clinical determination that HPM is no longer appropriate
  • Technical issues preventing effective use of the CS Connect app or HPM feature
  • Inability to obtain or maintain a suitable blood pressure monitoring device
  • Termination of your CS Connect virtual healthcare services
  • Patient request

Upon termination, your access to the HPM Program feature within the CS Connect app will be disabled.

9. LIABILITY AND DISCLAIMERS

9.1 Medical Advice Disclaimer

The HPM Program does not replace regular medical care, clinical judgment, or consultations. All medical decisions remain between you and your healthcare provider.

9.2 Technology Limitations and Third-Party Services

We do not guarantee uninterrupted service or error-free technology. Technical issues may affect data transmission, storage, and functionality of Program features, including but not limited to the Program’s alert tools or provider access to your information through the CS Connect app. The HPM Program operates as a feature within the CS Connect existing mobile application, and service availability may be affected by system maintenance, updates, or technical issues affecting the broader CS Connect platform. We are not liable for any interruptions or limitations in CS Connect’s services that may affect the HPM feature.

9.3 Limitation of Liability and Device Disclaimers

To the fullest extent permitted by law, we disclaim liability for any indirect, incidental, or consequential damages arising from your use of the HPM Program, except for damages resulting from our gross negligence or willful misconduct. We specifically disclaim any liability related to blood pressure monitoring devices, as the HPM Program does not provide these. Any issues with monitoring devices should be addressed with the device manufacturer.

The CS Connect Terms govern all other terms and disclaimers, including, but not limited to, technology disclaimers, service availability disclaimers, and general platform disclaimers.

10. ENTIRE AGREEMENT

These Terms, together with the CS Terms, the Informed Consent for Virtual Telehealth Services, and all other applicable consent forms and agreements related to your care through CS Connect and Cedars-Sinai, constitute the complete agreement regarding the HPM Program.

By accessing the HPM Program or otherwise indicating your assent by clicking I Accept, Continue, Next, or a similar button, you acknowledge that you have read, understood, and agree to be bound by these Terms.
